Please complete the form below to submit your order request. The request will be sent directly to the staff at Healing Family Functional Medicine, who will place the order on your behalf. You will receive an email from Tiny Health with a secure payment link to complete your transaction. Once payment is submitted, Tiny Health will ship the test kit to your home.
Please note: Test kits are not shipped until payment is received. Orders expire after 90 days if payment is not submitted. A reminder email is sent to the patient every seven days until the order is completed, canceled, or expired. A scheduled appointment is required to review results.
